List of administrative procedures performed in the health care institution “Brest Regional Psychoneurological Dispensary”

(Decree of the President of the Republic of Belarus No. 200 of 26.04.2010).

7.4. Issuance of a medical certificate of death (stillbirth)	Dispensary. The person who accepts an application for an administrative procedure is the attending physician, and the supervisor is the head of the department.	passport or other identity document of the deceased	person free	of charge on the day of application

7.5. Issue of a disability certificate (temporary disability certificate)	Dispensary. The person who accepts an application for an administrative procedure is the attending physician, and the supervisor is the head of the department.	passport or other identity document, medical documents (extracts from them) issued in a foreign country, medical certificate of health status, death certificate, travel documents, a permit for sanatorium treatment, a copy of the certificate of referral to work-in case of issuing a certificate of disability (certificate of temporary disability) at the address of the state of health of the Russian Federation. grounds that cannot be established during a medical examination, medical examination and need documentary confirmation	free	of charge on the day of establishing temporary disability

7.6. Issue of a medical certificate on the state of health	of the Dispensary. The person who accepts applications for administrative procedures is a psychiatrist-narcologist, a child psychiatrist. Control – head of the department.	passport or other identity document, an extract from medical documents (except for information about the absence of psychiatric and drug treatment records), two 30 x 40 mm photographs – for obtaining a medical certificate of health confirming the suitability for driving power-driven vehicles, self-propelled vehicles, small motor vessels with an engine power exceeding 3.7 kW (5 horsepower)	free	of charge 1 day after the medical examination, medical examination
The validity period of a certificate or other document (decision) issued (adopted) during the implementation of an administrative procedureis up to 1 year, and for a medical certificate on the state of health confirming suitability for driving power-driven vehicles, self-propelled vehicles, small vessels-up to 5 years
7.9. Issue of extracts from medical documents	Dispensary. The person who accepts applications for administrative procedures is a psychiatrist-narcologist, a child psychiatrist. Control – head of the department.	passport or other identity document	free	of charge 5 days from the date of application

An application for “Issuing an extract from medical documents” can be submitted in writing on the basis of an application addressed to the chief physician in the registry window to any of the medical registrars.

All administrative procedures performed by the organization can be performed by all doctors of the healthоcare institution.
